Yeah, It kind of went with his character then. It was like Rock did not even care enough to get into wrestling gear to face the "low life" called Mankind.

It also kinda fit the storyline. There was this whole idea that Mankind was so hardcore, the Rock would be taken to pieces. That the Rock was some jumped up nouveaux riche that only kept the title because of the Corporation. So he comes out in that tracksuit, and he looks like some idiot trying to look 'ghetto' and prepared for a hardcore match that no one thinks he's going to win.

Then he proceeds to kick seven shades out of Mankind and surprises everyone, solidifying himself as both the top heel and a genuine star.

The rock like many bodybuilders/ long term steroid users have to have their glands removed because breast tissue starts to formulate (gynocomastia) which he clearly had since a young age (15). Bodybuilders typically take anti estrogens on cycle but I guess rock was too young and ignorant on how to use steroids safely that he messed his chest up. So he had surgery to reduce his boobs.
