Is every single part in the rumble planned before the match?
Do all the participants know who should eliminate who, and when?
And what order everyone comes out in?
Obviously the major spots are planned, like the last 4 etc. But how much is called during the match?

I would imagine a lot is called out there, but they are given rough ideas. Order of elimination can just go with 'When you see Tyson Kidd, he is number 11 and you have to be out before 12 comes out" and so on, plus there are a lot of referees around to keep track of everything.

Spots I would suggest are planned too, ie Morrisons parkour in this years rumble and Hornswoggles moment, but not given a timescale.

Would be guessing tbh. I think if anything, they all know WHEN they've to go out. For example, I need you out before number fifteen is in - then they can do a call out with someone to make it happen based on what feels right.

Obviously there are exceptions to this rule - like Cena getting eliminated by Miz or whatever - or particular special spots like Morrisson's thing on the railing.

And I'd obviously bet that everything including the last four is pretty much scripted in detail.

So yeah, that's my take - they obviously can't just let the majority of guys design their own fate in the ring because egos can take over. Someone might try to make himself a big star, by refusing to be put out and lasting too long and taking out too many guys.
