Take a closer look and compare the muscle busters, you'll notice Joe typically tilts towards his right before slamming the opponent so that the pressure slides off to one side and doesn't hurt the neck and spine. However against Tyson, he completely failed to do that, falling flat on his back putting the entire pressure of the dangerous move on Tyson's neck region and his back injuring him badly in his debut match.

Notice how Tyson immediately wraps his left arm around his left side of his neck following the impact?



Also, for those who say "Joe has done this 1000 times!". You need to keep in mind the fact that back then Joe was still in his prime, in his 20s performing this move safely, but now he's old and much like an old washed up Kane, is no longer able to perform at his 100% prime self. He's way out of shape as well.
