It's happened but never after they announced the winner gets a title shot at WrestleMania. They had Chavo Guerrero enter as ECW Champion one year but the ECW title isn't a "real" world title, so that doesn't count.
I actually wouldn't be surprised if they did it with Ziggler this year. There's a Last Man Standing match at the Royal Rumble, a perfect opportunity to cash in, and his entry number is presumably locked already, because he won the beat the clock challenge. I don't expect them to be this clever but it COULD happen this year. I wouldn't bet on it though.

When WWE was struggling, it's Shawn Michaels and Bret Hart in the main event going 60 minutes, people are leaving during the main event of WrestleMania, you can check the video tape, and Hall and Nash have both told me that Vince said that night, that he would rather have a half full arena cheering for the guy that he says to cheer for, than a full arena cheering for whoever they want.
