Not the worst game by any stretch but one game that I can remember being disappointed in was Mario 2.
It didn't play at all like a mario game and was just boring as hell. I remember as a kid being really fucking disappointed in it. Thankfully just about every Mario game that has come out since has been mostly awesome.
Super Mario 2 was my favorite NES Mario game by far.
Worst game that I ever actually bought, was Cosmic Carnage for the 32x
Some Atari games I played a decade ago were just awful. Can't remember any names off the top of my head but I know Super Mario was awesome unlike the rest.

SVR 07 was awesome. The worst SVR is by far 2010. Roster is weak, moves are still lacking compared to 06 and 07 and it's just frustrating all around. The camera angle when leaving the ring sucks (trying to make it like real life backfired) and the fact that you/your opponent get up after being hit with a move in an INSTANT makes it even more annoying.
WWF Wrestlemania (NES) is beyond awful
I bought this appalling game for the PS1 called Chaos Break. It was a survival horror game (as far as I could tell) and the dialogue and voice acting made those in Resident Evil look Oscar-worthy.
The 3DS version of FIFA 12 is a huge steaming turd (aside from the Street mode which is semi-enjoyable).
FIFA 07 for the Xbox 360 was also terrible.
There was also a Resident Evil game which came out on the Game Boy Colour, which I think was called Resident Evil Gaiden. Barry Burton was the main playable character. Anyway, that was shit.
I'm sure there are others, but these stick out in my mind.
