Other than possibly falling down a mountain, there is no way Lydia could die without your input. You probably used an AOE spell/potion or swung your sword or have some cloak spell on or shot an arrow accidentally to her. I've sent Lydia to fight an ancient dragon while I stand behind and watch. Lydia goes "down" then she gets back up after a certain amount of time. I just got my platinum yesterday. Lydia never died in my entire playthrough without me accidentally hitting her and I was a lazy bum who let Lydia and my Dremora Lords do all the attacking.

^ I've just now gotten into the main quest and all my friends who played it look at me like an idiot. I've gotten to Level 42 with just side shit and other things.

It's really not fair sometimes. You want to do one mission, then someone presents you with another and you can't help but do that one since you might find something or gain more experience. Everything BUT the main story makes this game good.

The best thing you'll gain from the main quest is learning Unrelenting Force in its entirety. What a goddamn satisfying shout. There's just something about sending giants and mammoths flying in the air.

Haven't exactly gone any further myself either.

Indeed. Once I learned the entire shout I went on other missions just for the simple satisfaction of making shit fly. I don't even care that it takes forever to recharge, when there's a long fall over a ledge or a long hallway behind them you best believe I'm gonna Fus Ro Dah.

I use to favor the Fire shout but fuck that, I want my enemies to feel the ultimate GTFO

My favorite kill I ever made was on the catwalks in that one bandit cave Lost Knife Cavern (or whatever its called) and I unrelenting forced some fool across the cave causing him to bounce off the ceiling and fall to his death in the water.

The guy I saw get killed by a gate coming down on his head (to the point his body was still attached the gate when I reopened it) was a close second.

Finish the main quest asap. Dragonrend is so useful. Finishing the Civil War after going to the Throat of the World is a good idea tho.
