This is a graphics card problem, right?

Dusted off the old computer I had standing around, but sadly it just keeps crashing. Problems in specific ->

1) Graphics driver is crashing frequently, doesn't matter what I do. I can play Skyrim on Ultra for ~2 hours and nothing happens, then I'm chatting in Skype and I get the error reporting "NVidia graphics driver stopped responding, but got restored" (Its in german, so no idea how that error translates )

I tired reinstalling it and tried some older versions, driver keeps crashing no matter which one I use. Card is a Gefore GTX 750 btw

2) The sudden blackout
From time to time, my screen just goes black but everything seems to be running perfectly in the background (I get Skype message alerts for example, and when I'm playing music it just keeps going). This also seems to happen at random, during gaming but also when I'm just chatting.

I checked the temperature and its ok, even when I'm playing games it stays pretty cool.

What I tried ->

Reinstalling Windows
Reinstalling and updating every single driver on my computer
Reducing graphics and screen resolution

So, is the card nearing its end or is it something software related?

From what I made out it sounds like the issue only happens with Skype, if that's correct than yes it is indeed software related as you seem to be having no issues with other programs and games that require higher graphics use.

If all of Adobe is up to date (especially Flash player) then it does seem like your Nvidia card's drivers either need updating, your error report of 'stopped responding but ... restored' is a software issue more than the Card itself. Do video-players work for you or does YT etc black out also?
The other day my screen was blacking out on this site but when I checked my add-ons my Flash-player was outdated and needed the upgrade and as soon as I updated and reopened browser the black screen cleared.
