I think it's time for a major storyline. Punk's heel turn isn't good enough.

Joker/Bane Ambrose:
Dean Ambrose debuts...by savagely beating Cena with a lead pipe. He is "injured." I don't mean CHICKENSHIT "injured" and wrestling the next week. I mean substantially shelved for a full 3 months minimum where he also misses PPVs. It is NEVER explicitly stated or proven that he was the one who did it, he does it in a disguise and a mask but everybody in the company and all the viewers knows it was him. He leads a faction of all the disaffected, under-utilized heels that Cena and Sheamus no-sold on the mic and buried in the ring. RAW is Dean Ambrose's plaything and if you don't want to play...get out. Smackdown is controlled by the charismatic figures of Wade Barrett and Cody Rhodes, who as initiation to the Ambrose Collective lets Ambrose destroy his face and make him the Disfigured One again.
Not a clear mask, make it more like Mankind meets Doctor Doom.

WWE have had plenty of "controversial" or "revolutionary" storyline ideas but their problem is writing the story at any great length and the payoff doesn't live up to the hype of the initial angle. We as viewers want captivating television and story progression every week we watch the program but either the creative writing team and/or Vince fall short of delivering from start to finish. Look at the Nexus (2010), Punk's pipebomb (2011), Jericho's "It Begins" (2012), Lesnar/Heyman lawsuit(s) (2012) as examples. So we all can come up with fantastic ideas. WWE creative can come up with fantastic ideas but in the end it is the direction that Vince/Steph takes them that will determine the idea's worth in execution.

Wrestlemania 29. WWE and World Heavyweight Championship unification match. CM Punk vs Sheamus. In the middle of the match, all of sudden, an unknown music hits. Some man runs down to the ring, hits his finishers on both wrestlers and shop them. "Ladies and gentelmen, here is your winner... And NEW Undisputed WWE World champion... Dean Ambrose!"

Talk about controversial.

A Cena heel turn will only work if he's presented as being truly deplorable and evil.
My idea is that he becomes a coprophiliac, a man sexually aroused by faeces. And after every match he pulls down his jorts and takes a dump in the middle of the ring, give him TL Hoppers old theme and it'll be bigger than the NWO!
