First things first.
You need to do all you can to kick drugs.
Heroin isn't just a recreational drug. It's hard. It will fuck up your life.

I know it's not easy, but even buying enough to cover your growing fixation will leave you in a potthole of debt.
You cannot rely on drugs as a way out; find another outlet.

It maybe not be too earluy to hit rehab. Honestly; your 16 and your taking heroin?

That can't continue mate.

Also; I'm going to be brutally honest; it sounds like your brothers would be better with a foster family or in a care centre.

They need care, and they're not getting it at home. They have a severe disibility and need people to give them attention and time and it doesn't sound like either of your parents is capable.

As for your ex. Say to her face, how you feel. You need her out of your life. You can't choose your family; but you can sure as hell choose everyone else.

Get rid of her. NOW!

It doesn't sound like your in the right frame of mind to be dating at the moment either. Tell this to your current girlfirend. I'm sure she'll understand and it'll stop your ex harrasing her.
