A lot of continuity has been lost through the years with regards to the Undertaker character and the story, although saying that I do find it impressive that they've managed to keep the main points there and never destroyed the character completely. It's just little things that don't add up and in wrestling that is to be expected. In regards to the questions though:

The fire that killed his parents has been said to be unintentional at some points but then I seem to remember it being played up as being intentional during the ministry storyline? And then never really mentioned again.
Paul Bearer is Kane's father, he had an affair with Taker's mother after he was born, after the fire he took Taker under his wing. This makes Kane his half brother.
He was billed as being from Houston in his bad ass days but went back to being billed from death valley when he returned as the dead man.
He stopped being the bad ass after Kane buried him alive, when he returned the next year to face Kane he was the deadman again.
The whole idea behind the ministry angle to start with was Mark Calaway had become so involved in his character that he started believing he really was the Undertaker, which is why Vince called him Mark.
Yep, Sara was on tv too, that was the focus of the storyline at that point, she has also been involved in the feud with Brock Lesnar.
Contents of the Urn are still unknown as far as i'm aware.
Again the WWF/E has changed it's mind on who the Undertaker is a few times, they've made him out to be the character that he portrays outside of the ring too, then in the ministry angle they acknowledged it was just a character that Mark Calaway was playing, he left just after this storyline and this lead to him returning as the american bad ass.
Yep, the 'powers' they possess are still supposed to be real.
The Undertaker character was originally a kind of zombie/ghost - 'the deadman', that evolved to the satanic lord of darkness, that then evolved to the american bad ass, and then back to the deadman. For a long time in the ministry though he was booked as being satanic, though it later turned out the higher power was Vince so it is arguable that all of the satanic stuff was just an act (from the character's perspective).

What has always puzzled me is how they explain why Taker starts of as Satanic, from debut, to ministry, starts riding bikes and wearing bandannas in the Unholy Alliance stint that happened after the Ministry angle, which paved the way for the Badass. I dig, that if being buried alive, you're technically going to be dead when you return, but, a really significant part of the story of the Deadman's 2004 return, I think, is not the Wrestlemania match against Kane, in which he wore his hybrid deadman clothes, but in fact that now famous Smackdown episode, with Paul Heyman saying "And there's not a man alive, who will stand in my way" and outcomes a Deadman Undertaker, sporting Deadman entrance, and hybrid Deadman clothes, but also a bandanna. I think, Heyman's reaction of "Oh my god, help me!" is directly relating to the still puzzling use of that bandanna, which has never been seen on the Undertaker since.

During the ddp stalking angle they definately mentioned his wife sara numerous times. At summerslam 01 she accompanied kane and taker to the ring for the tag title match against the jersey triad (ddp and kanyon). Dead homeboy also had her name tattooed on his throat, im fairly certain he removed/changed the tattoo

The Undertaker since 98 would appear backstage in "badass" clothes. Wearing a bandana and shades.
Thus the American badass later was the same Taker, not a different one. And it made sense too, since we had seen that side of him.
Hell in a backstage segment during 98 they showed taker and Kane arriving to the arena and Kane was rocking a football jersey! (Raw episode, sometime between sumerslam - breakdown ppv)

Hey bro I watching the raw episode after the ppv cold day in hell and Paul bearer came out and threatened the undertaker that if he doesn't come back to him he will reveal his secret.
So maybe Paul kept Kane at bay all these years because he was with Taker. And when he saw that mankind couldn't get the job done of destroying him he went to the last resort: unleashing Kane.

Great thread, actually had a lot of things refreshed & learned a few things I didn't know or forgot completely.

They really fucked up the ministry angle IMO by acknowledging it as a gimmick though. Man I have fond memories of that time in wrestling but some of that Russo reality BS annoys the fuck out of me somtimes.

Damn right brother. Russo's crash tv booking is the easy way out of creative writing.
Anyone can do crash tv. Only creative writers can write a legitimate professionaly written story.

Was the urn they used in his recent feud with CM Punk meant to be the one from the start of his career?
