Dexter, the TV series was amazing from 1-4 very compelling, 5-6 were let downs, they did not carry on the quality, instead it felt depleted.
7-8 I was hooked again and did not want to stop watching, however the ending fell flat and I feel most fans wanted Dexter to be caught.

Remake it for US TV. Red Dwarf and Coupling, spring to mind.

Roseanne series finale retconning most of the show. Horrible. Traumatizing.

Well on the Network side: Cancelling Firefly and firing Dan Harmon.

On the actual creative side (not really worst ever, but things that annoyed me greatly):

House deciding to replace the original three.
Cheers deciding the up the Diana is an annoying bitch side of her character.
Frasier deciding to get Niles and Daphne together at the end of season 7. The show lost all its tension after that.
Buffy deciding to waste half of the shows last season on those annoying ass Potentials.
Angel getting rid of Lilah.
NCIS turning Ziva into Mary Sue.
Criminal Minds replacing JJ just to have to get her back a year later.
The Simpsons not ending after season 10.

I'm still fucking pissed over this. I love the first two movies and still watch them. I'll even watch the 4th one. But seriously, fuck Aliens Cubed. Killing Newt and Hicks right at the start ruins the rest of the movie for me.

These are not bad decisions in and of themselves. It's only in hindsight that they are called bad decisions because some of the movies sucked. T3, T4 and DH5 all sucked. Die Hard 4 was actually a pretty good movie.

Crystal Skull was a decent movie too. There are just too many people whining over aliens at the end. The premise of the movie was based on the real life crystal skulls, whose mythology involves aliens, so what the fuck did people expect it was going to be about? Indy Jones has always been about supernatural artifacts. What happened in the new one is no more far fetched than anything that happened in the first three.
