The Rumble winner will probably be the WHC challenger, because otherwise they've got throw the Rock in the Elimination Chamber, and there's not a chance in hell of that. The Rock may work a regular title match (rematch clause?) at that PPV, but not in the Chamber.
What I kind of think will happen is that Punk loses the title to the Rock at Rumble, then ends up winning the Rumble and challenging Sheamus for the WHC at WrestleMania. His reasoning for challenging Sheamus would be that he's using his rematch clause against the Rock at Elimination Chamber in a bid to become unified champion at WrestleMania.
